WORK PERMIT FORM

Any student who is under the age of 18 must have a valid working permit. The Working Permit Application is available in the Main Office. Parent signature and proof of age are required for the application. Forms for proof of age may include, birth certificate, drivers permit/license, or a passport . Both parent and student will need to bring this documentation to the High School in order to get a working permit. Please enter door #1 of the CMHS Main Office for assistance.

                Student Assistance Program

The Student Assistance Program offers support to students who are struggling with attendance, academics, or behaviors. The goal of the program is to reduce barriers by connecting students to services.  If you are concerned about a student and want to make a referral, please complete the form on the student you are concerned about by clicking on the link below.

Family link Family Survey

The School Climate Survey: Family obtains parent perceptions of school climate within the following five subscales: teaching and learning, school safety, interpersonal relationships, institutional environment, and parent involvement. This 21-item survey uses a 4-point rating scale (from Strongly Disagree to Strongly Agree). Parents, caregivers, or guardians of enrolled students complete the survey without training either on or off campus. The time to complete the survey is approximately 10-15 minutes.

Educational Trip

Vacations are not a lawful excuse in the Commonwealth of Pennsylvania. We do recognize Educational Trips and Tours limited to 7 days per school year. A request form (available at the office or online) must be received at least 5 days prior to the trip and approved by the principal. Any days requested beyond the 7 approved cumulative days will be coded as unlawful. When planning educational trips, we urge parents to schedule them during summer months or during our breaks so minimal negative educational impact is experienced by your child.
