CMHS HALL OF FAME
CMHS Athletic Hall of Fame Nominee Qualifications
An athlete may be nominated at any time beginning five years after the graduation date of his or her class. A coach may be nominated ate any time beginning five years after retirement from that sport. A team may be nominated any time beginning five years after the graduation of the yougest class participating in the sport. Nominations may include coaches, trainers, equipment managers and other person who have made outstanding contributions to athletes at Central Mountain High School. A Central Mountain High School Athlete or Coach may have the five year waiting period waived for nomination to the Hall of Fame if they have been inducted into a national Hall of Fame participated in the Olympic games or has wona PIAA Championship.

2025-2026 Extra Curricular and Interscholastic Handbook
PIAA Physical Form - Section 6 Initial Physical Form. Must be dated after May 1st of each year for the upcoming school year.
PIAA Health History Form - Section 5 of PIAA Form. Some physicians will require this information when completing the physical.
PIAA Dr. Recertification Form - Section 8 of PIAA Form. This needs to be completed when there has been an injury or illness since the initial physical and the athlete needs to be cleared by a doctor to return to play.

Online Application for Pass - Once this is submitted the pass will be processed and mailed to you.
Paper Application for Pass - The completed form along with copy of drivers license can be emailed to athletics@kcsd.k12.pa.us or mailed to KCSD Athletics, 86 Administration Dr, Mill Hall PA 17751. Once received the passes will be processed and mailed to you.

Student Game Worker Requirements: must be a KCSD Student at least 14 years of age. If you are under the age of 18 you must obtain working papers from the main office at the school.
All clearances and volunteer disclosure forms should be sent to the personnel secretary or uploaded to your application.

All coaching/volunteer PIAA certifications should be uploaded to the PIAA website. Make sure to associate the school who are coaching/volunteering with and notify the athletics secretary that have been submitted.
